The PCC is the link between patients and providers. Responsibilities Manage enrollment process from referral to post-care follow up. Communicate patient updates to PCP to ensure coordinated care. Research and connect patients with community resources. Facilitate appointment adherence. Provide ongoing support throughout the patients healthcare journey. Translation of documents into Spanish. Support other care coordinators with their processes. Handle administrative tasks such as filing, data entry, and managing correspondence related to patient care. Assist with emergency food distribution; place orders with the food bank, schedule pick-up, prepare boxes, organize distribution schedule and distribute food. Skills Strong organizational skills with the ability to manage multiple tasks simultaneously. Experience in providing administrative support, including scheduling and record management. Proficient in verbal and written English and Spanish Excellent in-person, telephone, and written communication skills to interact with patients and providers.
