Overview We are seeking a dynamic and experienced Home Health Clinical Manager to lead and coordinate comprehensive patient care services within a home health setting. Duties Oversee daily clinical operations of the home health agency, ensuring delivery of safe, effective, and compassionate patient care. Supervise and support nursing staff, therapists, and other healthcare professionals to promote professional development and maintain high standards of practice. Coordinate patient assessments, develop individualized care plans, and ensure timely documentation using electronic health record (EHR) systems such as Epic or Cerner. Monitor compliance with healthcare regulations including

HIPAA, NCQA

standards, and state licensing requirements. Lead utilization review and management processes to optimize resource use while ensuring quality outcomes. Facilitate communication among interdisciplinary teams, patients, families, and external providers to promote seamless care transitions. Conduct regular audits of medical records, medical documentation, and clinical workflows to uphold accuracy and regulatory adherence. Requirements Registered Nurse (RN) license with current state licensure. Extensive clinical experience in acute care settings such as ICU, emergency medicine, or hospital medicine; critical care experience preferred. Proven leadership or supervisory experience managing healthcare teams in home health or similar environments. Familiarity with EMR/EHR systems like Epic, Cerner, or Athenahealth; proficiency in medical terminology and ICD coding (ICD-9/10). Knowledge of diverse clinical areas including geriatrics, dementia care, behavioral health, pediatrics, neonatal ventilator management, and long-term care settings such as assisted living or nursing homes. Strong understanding of patient assessment techniques including vital signs, physical examinations, venipuncture, catheter care, medication administration, tube feeding, ventilator management, and infection control protocols. Ability to interpret diagnostic evaluations and perform complex case management functions for patients with developmental disabilities or chronic illnesses. Excellent team management skills with experience supervising nursing staff and coordinating multidisciplinary teams.
