Our Pulmonary Hypertension Clinic offers state-of-the-art diagnostic services and treatment solutions for patients who have pulmonary hypertension. Quality care, education, clinical research and emotional support are the cornerstones of our program. We strive to bring the latest advances in diagnosis and treatment of pulmonary hypertension to the East Tennessee region, while providing a personal and supportive approach to each patient's individual needs. This includes optimal outcomes with early diagnosis, up-to-date treatment protocols, and opportunity for participation in clinical trials. Pulmonary Hypertension (PH) Clinical Coordinator serves as a clinical resource and coordinator for outpatient pulmonary hypertension services. This role supports PH physicians, nursing staff, patients, and external specialty programs to ensure coordinated, high-quality patient care. The PH Clinical Coordinator is responsible for staff education, patient support initiatives, prescription management support, and collaboration with transplant and CTEPH programs while promoting excellence in pulmonary hypertension care.
Examples of Duties :
(List does not include all duties assigned) Patient Care & Clinical Coordination Establish and support a designated team of at least four nurses assigned to work with Pulmonary Hypertension physicians. Coordinate nursing staff coverage and scheduling needs for PH physicians. Serve as outpatient backup nurse for PH physicians on non-PH Clinic days to ensure continuity of patient care. Collaborate with physicians, nursing staff, specialty pharmacies, and ancillary departments to support comprehensive patient management. Assist in coordinating care plans and facilitating communication among members of the multidisciplinary PH team. Education & Staff Development Implement and coordinate biannual outpatient PH nursing staff in-service education programs. Provide education related to pulmonary hypertension therapies, medication administration, side effect management, and treatment protocols. Organize and facilitate quarterly patient education and support group meetings. Serve as a clinical resource for PH-related education and best practices. Prescription Management Support Serve as backup support for PH prescription management processes. Assist with prescription coordination, prior authorizations, specialty pharmacy communication, and medication access support. Maintain familiarity with prescription workflows by routinely managing prescriptions for designated PH physicians as assigned. Lung Transplant & CTEPH Coordination Serve as the central point of contact with transplant and Chronic Thromboembolic Pulmonary Hypertension (CTEPH) program coordinators at Vanderbilt University Medical Center (VUMC). Facilitate communication, coordination of records, referrals, and follow-up related to transplant and CTEPH patient care. Continuing Education & Professional Development Complete a minimum of 12 hours of Pulmonary Hypertension continuing medical education (CME) during each three-year accreditation cycle. Attend either the Pulmonary Hypertension Association (PHA) International Conference or Pulmonary Hypertension Professionals Network (PHPN) meetings during alternating years. Maintain active membership with the Pulmonary Hypertension Association. KNOWLEDGE, SKILLS, AND ABILITIES
Knowledge of pulmonary hypertension disease processes, therapies, and specialty medication management preferred. Strong understanding of clinical coordination and multidisciplinary patient care. Ability to communicate effectively with physicians, patients, nursing staff, specialty pharmacies, and external healthcare organizations. Excellent organizational, critical thinking, and problem-solving skills. Ability to prioritize responsibilities and manage multiple tasks in a fast-paced clinical environment. Strong patient education and interpersonal communication skills. Proficiency with electronic health records (EHR) and Microsoft Office applications. Ability to work independently and collaboratively as part of a multidisciplinary healthcare team. EDUCATION
High School Diploma or GED required. Current RN, RRT, or Pharmacist licensure/certification required. Prior pulmonary, critical care, specialty pharmacy, or pulmonary hypertension experience preferred. Experience in clinical coordination, patient education, or specialty care preferred WORK LOCATION
