We are seeking an experienced and compassionate Health & Wellness Director (HWD) to lead the clinical and wellness operations of our Assisted Living community. This is a pivotal leadership role responsible not only for clinical excellence and regulatory compliance, but also for strengthening a culture centered on respect, dignity, and resident choice. The Health & Wellness Director serves as the senior clinical leader in the community--guiding care delivery, developing the nursing team, and ensuring high-quality outcomes in alignment with Tennessee Assisted Living Facility regulations. The ideal candidate is both a skilled clinician and a strong culture leader--someone who leads with presence, coaches their team, and builds trust with residents, families, and staff. Key Responsibilities Clinical Leadership & Excellence Ensure all clinical care meets or exceeds state and federal standards Oversee medication management, infection prevention, fall reduction, skin integrity, and change-in-condition response Lead care planning, assessments, and individualized service plans Maintain accurate, timely clinical documentation and audit readiness Prepare for and respond to Tennessee Department of Health surveys Leadership & Operations Oversee daily Assisted Living and Memory Care clinical operations Maintain compliance with Tennessee ALF regulations and internal policies Lead quality assurance, audits, and performance improvement initiatives Round regularly throughout the community to stay connected to residents and staff Team Development & Coaching Hire, train, supervise, and develop nursing and care staff Build a stable, engaged clinical team focused on accountability and compassion Coach team members to observe, document, and escalate changes appropriately Promote a collaborative, respectful work environment--leading through mentorship rather than authority alone Resident-Centered Care & Culture Support a care model that prioritizes resident preferences, independence, and dignity Partner with care staff to deliver personalized care aligned with each resident's lifestyle and needs Serve as a trusted clinical resource for residents, families, physicians, and partners Model a calm, respectful, and neighborly approach in all interactions Memory Care Oversight Ensure compliance with Tennessee dementia care training and staffing requirements Oversee therapeutic programming and safety protocols Ensure individualized plans support cognitive and behavioral needs Risk Management & Safety Oversee infection control, safety programs, and emergency preparedness Investigate incidents and implement corrective actions What We're Looking For Education & Experience RN preferred; LPN considered with strong leadership experience Minimum of 2 years' experience in Assisted Living, Memory Care, or Long-Term Care Strong knowledge of Tennessee ALF regulations Demonstrated ability to lead teams, manage clinical systems, and drive outcomes Licensure Active RN or LPN license in the state of
