Job Description

Longevity Coordinator / Patient Care Coordinator Functional Medicine & Longevity Clinic — Full-Time About Us We are a forward-thinking functional medicine clinic specializing in longevity optimization and hormone replacement therapy. We take a root-cause, whole-person approach to health — helping patients feel and perform at their best through personalized protocols, advanced diagnostics, and ongoing, relationship-driven care. Our team is passionate about changing the way people experience medicine. The Role We are looking for a motivated, compassionate, and highly organized Patient Care Coordinator to serve as the primary point of contact for our patients from first inquiry through ongoing care. You will guide patients through their journey with us — from initial onboarding and lab coordination to protocol follow-ups and everything in between. This role is equal parts relationship management and operational coordination, and it sits at the heart of everything we do. No clinical background is required — we will invest in training the right person. What we can't train is warmth, ownership, and the ability to make a patient feel genuinely cared for. Key Responsibilities Act as the main point of contact for an assigned panel of patients, building trust and continuity throughout their care experience Guide new patients through onboarding — explaining our approach, services, and what to expect from their care plan Coordinate lab work, hormone panels, diagnostic testing, and follow-up appointments, ensuring patients are prepared and nothing slips through the cracks Work closely with our physicians and other clinical staff to communicate care plans, track protocol adherence, and relay patient concerns Review and organize incoming lab results and health data, routing to providers and following up with patients per clinical direction Proactively reach out to patients at key touchpoints — post-visit, post-labs, and mid-protocol — to maintain engagement and address questions Educate patients on HRT protocols, longevity programs, and available services in a clear, accessible, and compliant manner Maintain accurate and up-to-date patient records within our EHR and practice management systems Assist with referral coordination to outside specialists as needed Support the broader clinical and administrative team to ensure smooth day-to-day operations What We're Looking For 2+ years of experience in a patient-facing, care coordination, or high-touch client service role — experience in a functional medicine, hormone clinic, or wellness setting is a strong plus Excellent communication skills — warm, professional, and confident across phone, email, and in-person interactions Strong organizational skills with the ability to manage multiple patient timelines simultaneously Comfortable learning and working within EMR and practice management software (training provided) A proactive, solution-oriented mindset — you follow through, take initiative, and take ownership of your patients' experience Ability to handle sensitive medical information with discretion No clinical license required; prior experience in a medical or wellness environment is a bonus What We Offer A collaborative, mission-driven team environment 401(k) retirement plan Health insurance
