Full-time | Monday - Friday | 8 a.m. to 5 p.m. The Patient Care Manager is integral to delivering compassionate, high-quality home health services. By guiding clinical staff and fostering a collaborative environment, this role empowers teams to grow and perform at their best — directly enriching the patient experience. Through effective coordination, clear communication, and adherence to professional and regulatory standards, this leader drives a culture of excellence, safety, and trust — shaping quality outcomes and making a meaningful difference in the lives of patients and the professionals who serve them. Patient Care Manager - Key Responsibilities Provide clinical supervision to ensure patient care meets professional standards, agency policies, and regulatory requirements Coordinate and monitor patient care delivery Supervise clinical staff to ensure consistent, appropriate service Maintain clear communication with patients, caregivers, referral sources, and field and office staff Drive quality outcomes by meeting department goals and supporting improvement initiatives Model professionalism and organizational values in daily work Uphold compliance through required training, accurate reporting, and cooperation with audits Promote a safe work environment by following established safety protocols Support staff growth through coaching and continuing education Patient Care Manager Qualifications Registered nurse with current licensure to practice nursing in the practicing state. Previous experience in home care setting with two years management or supervisory experience, preferred. Knowledge of accepted professional standards and practice, Medicare Conditions of Participation, and federal, state, and local regulatory requirements
