Overall Responsibility This position is responsible for providing Rapid Rehousing (RRH) placements along with case management and supportive services promoting housing and economic stability for survivors of domestic violence. Knowledge of and experience in Housing First, Mental Health, Addictive Disorders, Benefits Eligibility and Domestic Violence, especially as it relates to the homeless is highly advantageous. Specific Responsibilities Perform intake, interview, and assessment to qualify client Provide case management services to program participants Assist with housing search, apartment application process and related work for move-in Assist program participants with mainstream benefits and other long-term stable sources of income Cultivate a working relationship with program participants to reduce barriers, gain resources that will ultimately help with permanent stable housing. Assist program participants to develop case plans to address short- and long-term needs that will lead to financial stability and permanent housing. Conduct monthly home visits to review goals, check-in status, etc. Conduct quarterly assessments of program participants as well as on the program in general Facilitate relationships between landlords (property management) and program participants Work closely with case managers, outreach workers, interns and volunteers to identify and access services and resources to help each participant meet their individual needs Ensure contract compliance with all documentation Maintain communications and documentation with required data systems (HMIS, CAS, etc.) Work with other agencies to maximize residents' outcomes and goals. Maintain a work schedule to meet program needs with adequate flexibility. The individual in this position will support the mission and values of the agency, providing survivors of domestic violence with services which are survivor centered, accessible, trauma informed, equitable and inclusive. Required Education, Experience and Skills Prefer a Bachelor's degree in Social Work, Psychology, or related field; or commensurate work experience Two years advocacy and case management experience, preferably in a domestic violence agency Knowledge of domestic violence Knowledge of community resources Ability to demonstrate culturally responsive, equitable and inclusive practices Proficient use of MS Office/Windows operating system Proven time management and organizational skills Ability to work collaboratively with others - both internally and externally Demonstrated relational and interpersonal skills Ability to maintain accurate, timely documentation of services. Valid driver's license and proof of liability insurance. Preferred Skills Demonstrated ability to assess, plan, facilitate, and evaluate options and services required to meet a client's identified goals Experience in crisis response and working with survivors of domestic/sexual violence trauma Knowledge and experience in trauma informed care Bilingual Spanish-English Experience providing financial management and planning education
