Job Description

Job Summary We are seeking a passionate and dedicated Wellness Coordinator to join our assisted living community. In this vital role, you will oversee the development and implementation of comprehensive wellness programs tailored to enhance the quality of life for our residents. Your leadership will foster a nurturing environment where residents feel valued, supported, and empowered. With your expertise in senior care, memory care, and health management, you will ensure that each resident's physical, emotional, and social needs are met with compassion and professionalism. Duties Develop, coordinate, and oversee personalized care plans that address residents' unique health needs, including Alzheimer's and dementia care. Supervise and support staff members involved in resident care, ensuring adherence to safety protocols, medication administration, and quality standards. Collaborate with nursing staff, social workers, and family members to monitor residents' progress and adjust care strategies accordingly. Lead wellness activities and programs designed to promote physical activity, mental stimulation, social engagement, and overall well-being. Manage operations related to health services within the community, including budgeting, resource allocation, and compliance with regulatory requirements. Conduct regular assessments of residents' health status and coordinate necessary interventions or referrals to healthcare providers. Maintain detailed documentation on resident care plans, incident reports, medication logs, and other essential records with accuracy and confidentiality. Experience Proven experience working in assisted living facilities or nursing homes with a strong background in senior care or long-term care environments. Demonstrated supervisory or management experience overseeing caregiving teams or group home staff. Extensive knowledge of Alzheimer's disease, dementia care practices, memory care strategies, and resident safety protocols. Familiarity with developing and managing care plans tailored for individuals with developmental disabilities or requiring specialized support. Experience in operations management within healthcare settings including budgeting, resource planning, and compliance oversight. Background in social work or related fields that emphasize resident-centered approaches to wellness and community engagement. Prior home & community care experience is highly desirable for understanding diverse resident needs across different settings. Pay:
$23.00 - $25.00 per hour
