Job Description
Seaxon Home Health Care Agency is seeking a Home Health Office Case Coordinator to support daily patient care coordination and office operations. This is a non-RN, office-based position . An RN license is not required . The ideal candidate is organized, dependable, and comfortable communicating with patients, families, clinicians, and physician offices. This role helps make sure patient care is coordinated, visits are followed up on, documentation is tracked, and communication between the office and field staff is clear and timely. Home health experience is preferred, but we are willing to train the right candidate who has strong communication, organization, and follow-up skills. Duties Communicate with patients, families, clinicians, and physician offices regarding home health services. Help coordinate patient care by supporting scheduling, follow-ups, and communication between office and field staff. Track visit updates, missed visits, pending documentation, and patient-related follow-up needs. Assist with obtaining and tracking physician orders, signed documents, and care coordination notes. Help identify delays in care, scheduling issues, or missing information and report them to the appropriate team member. Maintain accurate notes and communication logs in the agency's system. Support intake, scheduling, clinical, and authorization teams as needed. Follow HIPAA and agency privacy standards when handling patient information. Help ensure patients receive timely and organized care. Requirements Previous healthcare office experience preferred. Home health experience strongly preferred. Strong communication and phone skills. Excellent organization and follow-up skills. Comfortable working with patients, families, clinicians, and physician offices. Ability to manage multiple tasks and priorities in a busy office environment. Basic computer skills required. EMR or healthcare software experience preferred. Knowledge of HIPAA preferred. High school diploma or equivalent required. Bilingual English/Spanish .
