Job Description:
InGenesis is seeking a Licensed Vocational Nurse (LVN) - Patient Coordination Cell (PCC) to play a critical role in supporting patient flow, discharge operations, and inpatient transportation at Brooke Army Medical Center. This position ensures the safe movement of patients, timely completion of discharge tasks, and effective communication between hospital units, clinical staff, and support teams. The LVN PCC may support discharge coordination, patient transport, or dispatch functions depending on operational needs. Key Responsibilities Discharge Coordination Support the creation and management of the daily discharge priority list in collaboration with nursing leadership. Review patient discharge status in MHS Genesis and verify information with unit charge nurses and care teams. Assist with discharge preparation, including vital signs, transporting patients, retrieving medications, and coordinating final needs before discharge. Maintain accurate documentation in MHS Genesis and internal tracking systems. Provide consistent communication with clinical staff regarding discharge delays, status updates, and completion of required tasks. Inpatient Transportation Safely transport patients, medical equipment, and personal belongings to assigned locations throughout the facility. Ensure patient safety practices are followed, including proper use of side rails, wheelchairs, and transport equipment. Provide courteous and compassionate support to patients and families during movement. Communicate changes in patient condition or concerns to clinical staff immediately. Adhere to all safety, infection control, HIPAA, and hospital compliance standards. Dispatch & Coordination Receive, triage, and assign transport requests to transport team members. Track request status, maintain communication with units, and troubleshoot delays or issues. Monitor transporter locations and ensure efficient patient flow through prioritization. Maintain accurate data entry for transport requests and end-of-day reporting. Serve as the primary point of contact for daily transportation operations. Operational Duties Perform opening tasks such as preparing workspace, charging devices, printing necessary documentation, and setting up communication systems. Complete closing duties including securing equipment, finishing data entry, organizing work areas, and locking sensitive documents. Required Qualifications Education Graduation from a State Board of Nursing-approved LPN/LVN training program, OR Completion of equivalent military training that permits eligibility to sit for the state licensure exam. Licensure Must possess a current, active, valid, and unrestricted LVN license from any U.S. state, the District of Columbia, or U.S. territory. Certifications Basic Life Support (BLS) certification through an American Heart Association or Red Cross-approved training program (must remain current). Advanced Cardiac Life Support (ACLS) certification (must remain current). Experience Minimum of one (1) year of full-time LVN experience within the last two (2) years. Experience must be in a Medical or Surgical inpatient unit. Must have at least one (1) year of concurrent experience in the specific type of unit assigned within the past two years.
