Full Job Description General Summary:
This position is primarily responsible for providing personalized and responsive quality care in a productive and cost-efficient manner and support team members, patients and administration with facilitating patient care processes.
Supervisory Responsibilities:
This position does not have supervisory responsibilities.
General Requirements:
All duties performed will be done accurately and in a timely manner. Ensure customer service is always maintained at the highest level. Exercise tact and courtesy when dealing with patients, visitors, providers, and co-workers. Must always adhere to customer service expectations including in-person and virtual (via telephone, or telehealth applications) communication. Participate in educational programs as needed. Other duties as assigned.
Essential Job Responsibilities:
Core duties and responsibilities include the following. Other duties may be assigned. Coordinate patient appointments, consultations, and follow-up visits. Collect and verify patient information, including medical history Act as a liaison between the patients and the medical team Work closely with providers to ensure seamless care for patients Coordinate referrals to specialist or other services when needed Maintain accurate patient records, including medical history, appointments, and treatment updates Track patient progress and assist in the quality improvement initiative by reporting on outcomes and trends Ensure all medical records are up-to-date, accurate, and readily available for the healthcare team Prioritize patient needs and ensure timely care delivery in a fast-paced environment Ensure customer service is always maintained at the highest level. Ensures clinics compliance to company policies and procedures. Ensures compliance with Regulatory Agencies' guidelines. Assist patients with problems or concerns as may arise. Maintains professional affiliations and enhances professional growth and development to keep current in the latest trends in collections management. Maintains strict confidentiality Other duties as assigned.
Education and Training:
A graduate with a High School degree or military equivalent training required. Knowledge of case management, quality management, utilization management and risk management preferred. Must be able to communicate effectively in both written and verbal forms.
Experience:
Experience in a clinic, doctor's office or hospital with a minimum of one-year experience preferred.
Other Requirements:
Computer Skills :
Skilled in use of computer/EMR systems. Knowledge of Word processing software, spreadsheet software, Internet, and database software. Required to provide bed-side nursing when needed or during peak volume hours.
Work Environment:
Clinic environment. Exposure to communicable diseases, bodily fluids, toxic substances, ionizing radiation, medicinal preparations, and other conditions common to a clinic environment. Mental /
Physical Requirements:
Requires manual dexterity, sitting, standing, stooping, reaching, kneeling, crouching, bending, walking, lifting up to 40 lbs. without assistance. Close vision and ability to adjust focus. Must be able to work efficiently under pressure.
