This role focuses on reducing barriers to care, increasing awareness of available resources, and supporting continuity of care for underserved or at-risk populations. Key Responsibilities Develop and maintain relationships with community partners (schools, healthcare providers, nonprofits, faith-based organizations, and local agencies) Conduct outreach activities such as workshops, presentations, and community events to promote mental health awareness and available services Identify individuals in need of outpatient mental health support and provide appropriate referrals Assist clients in navigating access to care, including scheduling appointments and connecting to resources (housing, transportation, insurance, etc.) Collaborate with clinical teams to ensure smooth intake and continuity of care for referred clients Track outreach efforts, referrals, and outcomes; maintain accurate documentation and reports Advocate for mental health awareness and reduce stigma within the community Stay informed on local, state, and federal mental health resources and policies Support crisis response efforts by connecting individuals to appropriate emergency or urgent care services when necessary Qualifications Bachelor's degree in Social Work, Psychology, Public Health, or a related field (Master's preferred) 1-3 years of experience in mental health, community outreach, case management, or related work Strong knowledge of mental health resources and outpatient care systems Excellent communication, interpersonal, and presentation skills Ability to build trust and work effectively with diverse populations Strong organizational and time management skills Experience with data tracking, reporting, and basic computer applications Valid driver's license and ability to travel locally as needed Preferred Skills Bilingual or multilingual abilities Experience working with underserved or high-risk populations Knowledge of trauma-informed care and culturally responsive practices Familiarity with electronic health records (EHR) systems Work Environment Combination of office, community-based, and field work May require evenings or weekends based on outreach events Local travel required Key Competencies Empathy and cultural sensitivity Collaboration and partnership-building Problem-solving and resourcefulness Advocacy and community engagement Adaptability and initiative
