As a Patient Care Coordinator, you will play an essential role in helping patients feel heard, supported, and cared for throughout every step of their journey with us. You will serve as one of the first points of contact for patients, helping create a positive and seamless experience from scheduling appointments to coordinating follow-up care. Responsibilities Patient Check-Ins & Scheduling Conduct patient check-ins for scheduled appointments Verify and update patient demographic and insurance information Assist patients with both in-person and telehealth appointment procedures Schedule new patient appointments and follow-up visits Reschedule patients who cancel or no-show appointments Provide appointment reminders and assist with coordination of care Insurance Verification & Billing Support Verify patient insurance eligibility and coverage for in-person and telehealth services Communicate insurance details, copays, and coverage limitations to patients Assist patients with insurance-related questions or concerns Collect copays, outstanding balances, and other patient payments Coordinate with the billing department to support accurate billing and invoicing Follow up regarding outstanding balances and payment arrangements Documentation & Administrative Support Maintain accurate and detailed patient records in compliance with HIPAA regulations Update electronic health records (EHR) and ensure documentation accuracy Document patient encounters, communications, and treatment-related information Upload incoming faxes and distribute documents to designated personnel Respond promptly and professionally to emails, phone calls, and patient messages Manage incoming and outgoing correspondence Patient & Provider Communication Coordinate communication between patients and healthcare providers Communicate medication refill requests and patient concerns to providers Help ensure timely follow-up care and continuity of treatment Educate patients on office procedures, telehealth expectations, and appointment preparation Provide compassionate support to patients while maintaining professionalism and confidentiality Qualifications High school diploma or equivalent required Previous experience in a healthcare setting preferred, especially in patient care coordination, front desk, or administrative support roles Experience with scheduling software, electronic health records (EHR), and general office systems Excellent verbal and written communication skills Strong organizational skills with the ability to multitask and prioritize responsibilities effectively Attention to detail and accuracy in documentation and data entry Ability to handle sensitive patient information in compliance with HIPAA regulations Ability to work both independently and collaboratively within a team environment Compassionate, patient-focused mindset with a genuine desire to support others Working Conditions Monday, Tuesdays, Thursdays schedule with 2-4 hours every Friday On-site presence is required during weekdays Fast-paced healthcare office environment requiring multitasking and strong time management skills Friday shifts are 8AM to 12PM and then have
