Schedule:

Monday-Friday, 8:00 AM-5:00

What You'll Do as a Case Manager Conduct client assessments and gather essential background information Develop individualized care plans based on client needs Provide ongoing support, guidance, and resources to help clients achieve their goals Coordinate with clinicians, therapists, and other healthcare providers Maintain accurate documentation and case notes Build strong rapport with clients while offering consistent, compassionate care Collaborate with a dedicated onsite team to support both clients and clinicians Qualifications Applicants must meet ONE of the following:

Applicants under the "working toward SSW" pathway must obtain their SSW license within 24 months of hire. Associate's degree or equivalent and working toward SSW licensure Bachelor's degree with Social Service Worker (SSW) license Bachelor's degree in any field with willingness to pursue SSW licensure

Additional Requirements:
