- Full-time ACT Generalist Clinical Staff Position Description Under the supervision of the ACT Team Leader, the Generalist Clinical Staff will carry out rehabilitation and support functions as part of the ACT Team.
Incumbent will participate in collaborative client assessment and treatment planning. Incumbent will provide services in the client's home and in the community to support integration and stabilization. All services are provided as part of a comprehensive treatment team. This position involves rotating on-call duty overnights and holidays. Qualifications Appropriate candidates must meet the following qualifications:
- Bachelors degree in a relevant field with professional, clinical experience in the provision of substance abuse treatment or a combination of education, training, and experience indicating the possession of essential knowledge, skills, and abilities.
- License or certification as LMHP, QMHP-A, QMHP-E, or QPPMH is required.
- Experience in the delivery of mental health/case management services to adults with serious mental illness required.
- A valid Virginia driver's license and the ability to travel independently to a consumer's residence, work site and/or hospital setting required.
- The ability to be on-call and provide emergency response required.
- Ability to communicate and consult effectively with other healthcare professionals.
Position Responsibilities 1. Displays commitment to the goals of ACT, the ACT philosophy and the team's response to the needs of the consumer. 2. Serves as a member of the ACT team providing on-going case management, medication monitoring, life-skill development, supportive counseling, crisis intervention, and/or specialized services consistent with training/certification to ACT consumers as warranted and/or requested. 3. Establishes and maintains a treatment relationship with the consumer providing face-to-face services to the consumer in the home, the community, and with other agencies and family as appropriate. 4. Participates in the development of individualized treatment/recovery/rehabilitation/wellness plans, overseeing the identification of resource development and delivery of appropriate clinical specialty services. 5. Coordinates and provides ACT Team training and development in areas of clinical and/or management specialty. 6. Provide direct and emergency back-up service support to members of the ACT team. 7. Provides available on-call services on a 24-hour 7-day per week basis. 8. Responsible for accurate and complete documentation of provided services in accordance with established policy guidelines for record keeping. 9. Participates in on-going supervision and/or program development with the Team Leader which may consist of field visits to the consumer's residence, work site, family, etc. 10. Participates in the ACT Team's daily planning and coordination meetings, treatment planning conference meetings, etc
