Job Description

Home Health Clinical Director Position Summary Lamoille Home Health & Hospice is seeking a mission-driven, collaborative, and visionary Clinical Director to lead and oversee clinical operations across our home health programs. The Clinical Director will ensure the delivery of high-quality, patient- and family-centered care in the home and community setting while maintaining full compliance with CMS Conditions of Participation and applicable Home Health regulations, state regulations, and organizational standards. The ideal candidate demonstrates strong clinical leadership, regulatory knowledge, and a deep commitment to compassionate, community-based care. This role partners closely with the Hospice Director and Executive Director and interdisciplinary leadership to support staff, drive quality outcomes, and advance our mission of serving our community with dignity, respect, and excellence. Key Responsibilities Clinical & Operational Leadership Provide strategic leadership for clinical services across home health programs. Ensure care delivery reflects Lamoille Home Health & Hospice's mission, values, and commitment to patient and family-centered care. Support and guide interdisciplinary teams including nursing, therapy, social work. Ensure compliance with CMS Conditions of Participation, including 42 CFR §418 (Hospice) and applicable home health regulations. Maintain readiness for state surveys and regulatory reviews through ongoing quality monitoring and staff education. Oversee clinical policy development and updates aligned with federal, state, and accreditation standards. Monitor clinical outcomes, patient satisfaction, and safety indicators. Implement evidence-based improvements to enhance quality, efficiency, and patient experience. Foster a supportive, collaborative clinical culture focused on accountability, growth, and continuous learning. Support recruitment, retention, and workforce development initiatives. Qualifications Licensed Registered Nurse (RN) preferred; other appropriate clinical licensure considered. Minimum of 2 years of progressive clinical leadership experience in home health, hospice, or community-based healthcare preferred. Strong knowledge of CMS regulations, including 42 CFR §418 Hospice Conditions of Participation . Demonstrated experience with quality improvement, regulatory compliance, and survey readiness. Proven ability to lead interdisciplinary teams in a patient-centered care environment. Excellent communication, leadership, and organizational skills.
Pay:
$85,000.00 - $110,000.00 per year
