Care Manager (In‑Home Care)
Full-Time:
$24 - $37 DOE We are seeking individuals who thrive in a fast-paced, constantly busy, and always changing environment. About the Role The Care Manager oversees services for assigned care recipients and takes ownership of the quality-of-care families rely on every day. This role is ideal for someone who thrives in a fast‑paced, high‑pressure environment and finds real satisfaction in the impact they make in the lives of seniors, their families, and the caregivers who support them. Care Managers are the primary point of responsibility for care planning, caregiver performance, and client satisfaction, and often build meaningful relationships with families who genuinely appreciate the care and support being provided. This role requires independent judgment and is not intended as an entry‑level position. Key Responsibilities Conduct in‑home assessments and develop individualized care plans Be willing to perform common caregiving tasks including non-weight bearing transfers, shower assist, and incontinence care Build and maintain strong relationships with clients, families, and caregivers Match caregivers to clients based on needs, skills, and our staffing philosophy Supervise caregivers through ongoing communication, feedback, and performance evaluations Ensure compliance through accurate documentation and regular home visits Collaborate with the care team to coordinate services and triage emerging needs Participate in a rotating schedule for after‑hours weekday and weekend support What We're Looking For Self‑starter who takes initiative and solves problems quickly Comfortable in a fast‑paced, unpredictable work environment Able to demonstrate a consistently positive and engaging approach Clear, professional communicator with strong people skills Computer‑savvy and efficient with documentation and systems Able to navigate conflict with professionalism and empathy Strong alignment with values of dignity, respect, and teamwork Experience as an NAC, HCA, CNA, LPN, caregiver, or caregiver supervisor
