Summary of Patient Care Coordinator:
The Patient Care Coordinator runs the front desk, checking an average of 200-250 patients per day, while verifying insurance, patient demographics and scheduling future appointments. This position works under the general direction of a physician or their designee, utilizing communication and management skills to coordinate seamless care delivery for patients in a surgical or non-surgical setting. Requirements for
Patient Care Coordinator:
1+ year of experience using Epic (EMR) system 1+ year of recent experience working in a high-volume outpatient clinic or hospital setting 2+ years of patient services experience Experience with patient registration processes (150 Prior experience verifying insurances / benefits plans Solid understanding of medical terminology desired Solid understanding of CPT and ICD10 coding Working knowledge of insurance authorization process Ability to effectively communicate both in writing and verbally, as well as to interact in a professional manner with colleagues, patients, families, and visitors Must be able to work well with all members of the health care team and in a team environment Must be flexible and willing to take on increasing responsibility Please note: Candidates without Epic (EMR) experience cannot be considered.
Responsibilities of Patient Care Coordinator:
Act as contact for clinic providers, patient and family, referring physicians, outside agencies, outside staff and general public by providing information regarding the clinic's services Act as liaison between the patient and the various clinic areas and ancillary services to coordinate activities involved in pre-operative planning and preparation or planned hospitalization Coordinate pre-admission tests, pre-surgery appointments/tests, and procedures as delegated by the provider or nurse Ensure a complete medical record is present at the time of patient visit, admission, and/or procedure Facilitate the flow of documents between patient, providers and payers Communicate with health care team members regarding the status and/or completion of care coordination activities If working in a specialty clinic that receives internal referrals, ensure that referrals are addressed in a timely manner under the direction of the clinic leadership team Work with financial services and establish procedures to ensure financial information is complete prior to appointments, procedures or admissions Assist with scheduling surgeries and/or procedures or services needed in the ambulatory management of the patient's care Promote and protect general safety for patients/families/friends and co-workers through adherence to correct procedures, timely communication of messages Provide summary reports to clinic leadership on referral volumes and status (where applicable) Support development of clinic and divisional efficiency and effectiveness through participation in quality assessment and improvement activities Maintain/improve own professional competence, knowledge and skills to meet identified self-learning needs and annual review requirements Apply appropriate policies, procedures, protocols and standards of practice in performing duties Promote the mission of the facility externally in a positive way; function as an ambassador for the facility when interfacing with anyone outside the organization Demonstrate the team's values including patient-centered care and service; respect, caring and compassion; teamwork and partnership; continuous learning and improvement; and leadership at all times Consistent use of AIDET system for excellent service Aya Healthcare is an equal opportunity employer and encourages all to apply!
