On-site, patient-facing role Position Overview InSync is seeking a Patient Care Coordinator (PCC) to support a primary care clinic in Shoreline, WA. The Patient Care Coordinator plays a key role in ensuring seamless patient care by coordinating services, supporting providers, and serving as a patient advocate throughout the healthcare experience. This position requires strong communication, organization, and customer service skills while working closely with patients, families, providers, insurance teams, and healthcare staff to support efficient clinic operations. Essential Duties & Responsibilities Serve as a primary point of contact for patients, families, providers, referring offices, outside agencies, and the public by providing information about clinic services. Coordinate communication and care activities between patients, providers, and various healthcare departments. Assist with pre-appointment, pre-procedure, and care preparation activities as assigned. Ensure medical records and required documentation are complete and available for patient visits and procedures. Facilitate the exchange of documents between patients, healthcare providers, and insurance payers. Communicate updates and care coordination status with members of the healthcare team. Process and track referrals to ensure timely completion and follow-up. Coordinate scheduling of procedures, services, and appointments as needed. Work with financial services teams to ensure required patient information is complete prior to appointments or procedures. Maintain accurate documentation while protecting patient confidentiality. Support clinic efficiency initiatives and participate in quality improvement activities. Follow established policies, procedures, and healthcare standards. Provide exceptional patient service through professionalism, compassion, and effective communication. Demonstrate strong teamwork and collaboration with providers, staff, patients, and families. Required Skills & Qualifications Minimum of 1 year of experience in a medical or healthcare setting or 2 years of customer service experience. Experience providing excellent customer service in a fast-paced environment. Ability to handle confidential information with professionalism and discretion. Strong verbal and written communication skills. Excellent organizational skills with the ability to manage multiple priorities. Ability to work effectively in a team-based healthcare environment. Professional, patient-focused approach with strong interpersonal skills. Ability to build positive working relationships with patients, providers, and staff. Proficiency with Microsoft Office applications, including Word, Excel, and Outlook. Preferred Qualifications Experience working with Electronic Medical Records (EMR) systems. Knowledge of medical terminology. Familiarity with CPT and ICD-10 coding. Understanding of insurance authorization processes.
Pay:
$24.00 - $26.00 per hour
