Care Manager

Transitions At Home, is currently seeking to hire a Full-Time Care Manager to lead our Non - Medical Division in our Elkhorn location . What does a Transitions at Home Care Manager do? The Care Manager is responsible for enrolling new clients for services, training the Caregivers and performing services for the Client as necessary to maintain the personal care and comfort of the Client, while in their own home. The Care Manager will spend approximately 25% of their time treating our clients. This is performed in accordance with the plan of care under the direction and supervision of the Administrator.

Job Requirements:
