Client Resource Manager Location:
Sacramento, CA | Full-Time We are seeking a dedicated Client Resource Manager to join our care coordination team. In this role, you will be responsible for managing client relationships, coordinating care plans, and ensuring our patients and their families receive the highest level of service and support.
About First Place Pediatrics:
At First Place Pediatrics, we specialize in private duty nursing for medically fragile children. Our mission is to provide exceptional in-home care that helps children thrive in the comfort of their own homes. As part of our team, you'll play a vital role in ensuring continuity of care and strong communication between families, caregivers, and our internal teams. The CRM supports our back-office operations, allowing our staff to focus on delivering outstanding care to every family we serve.
Key Responsibilities:
Serve as the primary contact for clients and families Conduct intake assessments and develop care plans based on client needs Participate in KPI (Key Performance Indicator) meetings Educate families on home care services, expectations, and procedures Support caregivers with scheduling, communication, and guidance Maintain accurate client records, care plans, and documentation Ensure compliance with healthcare regulations and company policies Monitor quality of care and address service concerns promptly Assist with hiring, onboarding, and caregiver orientation
Qualifications:
Experience in home care, healthcare, or client-facing roles required High school diploma or equivalent required; associate's or bachelor's degree preferred Strong organizational, communication, and interpersonal skills Ability to manage multiple priorities in a fast-paced environment Knowledge of HIPAA and other healthcare regulations Proficiency with scheduling tools, EHR systems, and general office software Compassionate and professional demeanor
