The Population Health Manager serves as a critical bridge between strategic quality leadership and frontline operations at Promise Healthcare (PHC). Reporting directly to the Director of Population Health, this role oversees the day-to-day coordination of population health initiatives, provides direct supervision to care coordinators and interns, and serves as the dedicated Program Manager for assigned department initiatives, such as applicable grants. This position balances supervisory responsibilities with hands-on patient outreach. At the direction of the Population Health Director, the Population Health Manager ensures the execution of activities designed to close care gaps, meet clinical performance metrics, and achieve organizational goals for quality improvement (QI) and value-based care.
ESSENTIAL DUTIESRESPONSIBILITIES
Personnel SupervisionGrant Management Supervise, mentor, and evaluate department coordinators, ensuring accountability, proper clinical tracking, and effective patient engagement workflows. Recruit, train, and oversee department interns, aligning their learning opportunities with frontline population health and QI initiatives. Serve as the Program Manager for assigned department grants, managing operational timelines, supervising grant-funded personnel, and ensuring data collection fulfills federal and funder reporting mandates. Frontline OperationsPatient Outreach Actively participate in direct patient outreach alongside the coordination team to support strategic departmental goals and manage surge capacity. Oversee and execute comprehensive outreach, risk stratification, and care coordination protocols across multiple priority populations, including but not limited to: PerinatalChild Health:
Tracking confirmed pregnancies, monitoring early prenatal entry, and managing newborn-to-age-2 milestone and immunization compliance. Clinical Quality Measures:
Driving initiatives targeting Uniform Data System (UDS) eCQMs, HEDIS, and payor Pay for Performance (P4P) programs. Chronic CareNutrition Alignment:
Collaborating on workflows for adult BMI screening follow-up plans, controlled hypertension, and glycemic status assessments. Transitions of Care:
Coordinating post-hospitalization and emergency department follow-ups. Data, Quality Improvement,Workflows Utilize population health analytics, NextGen EHR, and payor registries to monitor patient panels, identify care disparities, and generate actionable outreach lists. Apply Plan-Do-Study-Act (PDSA) methodologies to refine internal referral paths, improve outreach success rates, and optimize staff workflows. Partner with the Director to develop, maintain, and present departmental dashboards tracking quality care metrics to clinical teams and leadership. Coordinate with case management and integrated behavioral health teams to ensure streamlined patient navigation and robust internal cross-referrals. Other Responsibilities Support intern program by providing mentorship and training to align with department goals. Assist with patient engagement activities, such as portal enrollment, informational events, and satisfaction surveys. Participate in staff meetings, training sessions, and interdisciplinary care team discussions as assigned. Support Promise Healthcare's mission, vision, values, and quality improvement plan. Perform other related duties as assigned. QUALIFICATIONS
Exceptional organizational, project management, and time management skills, with a proven ability to manage staff and multiple complex workflows simultaneously. Strong critical thinking skills with the ability to work independently and lead collaborative, interdisciplinary teams. Proficiency in electronic health records (NextGen preferred) and data reporting platforms, with an absolute commitment to data accuracy and clinical integrity. Excellent interpersonal and communication skills, demonstrating outstanding customer service, empathy, and professionalism when interacting with diverse patient populations, staff, colleagues, and partners. Must exhibit deep cultural sensitivity and comfort working in an inclusive health safety-net environment. Possess initiative, flexibility, and the ability to follow organizational protocols. Ability to adapt to changing work environment and duties as needed/assigned. EDUCATION / EXPERIENCE REQUIREMENTS
Associate's degree or equivalent required; bachelor's degree in social work, public health, health administration, nursing, or related field preferred. Minimum of 5 years of experience in a Federally Qualified Health Center (FQHC) or community health safety net setting preferred. At least 3 years of demonstrated experience in a supervisory, team-lead, or programmatic management role. Strong proficiency with NextGen EHR is highly desired. Prior experience with grant oversight, project reporting, and quality improvement tools (e.g., PDSA cycles) preferred. AMAZING BENEFITS
