Erie's Manager of Patient Programs
  • Population Health provides leadership, direction and strategic vision for quality initiatives across the organization to support UDS, HEDIS, agency and payor-related quality programs.
The Manager considers the unique needs and challenges faced by Erie's patient population and seeks to advance health equity, improve health outcomes, and ensure accurate reporting. Retirement Programs:

  • Compensation is based on each candidate's experience, skills and education within the range identified for the role.
Candidates who meet the minimum requirements of the role will start at entry in the range. Any additional skills, experience and education will be reflected in the compensation offered. Main Duties & Responsibilities Supervision Manages team schedules and time off requests. Completes hiring, training, scheduled performance evaluations, and 1:1 meetings with team members. Ensures implementation of agency policies and procedures. Facilitates regular meetings and trainings for the team. Program Strategy and Growth Leader of NCQA content at Erie Family Health Centers, including EHR-related content, reporting, and implementation of standards. Maintains project management role and core PCMH and NCQA expertise. Coordinates all activities, communication, updates, meetings related to NCQA. Quality Assurance, Budgeting and Reporting Outcomes Through reporting and chart review, identify process gaps, trends and disparities related to UDS and HEDIS performance metrics, as well as misalignment of payor and agency data. Provide oversight and ongoing management of care gap reports. Support leadership with analytics regarding financial earning potential related to HEDIS, ongoing tracking and trending of performance indicators/metrics, and reporting financial opportunities. Other Duties as Assigned.
Qualifications Education Required:
High school diploma or equivalent, with six (6) years of healthcare experience; OR Associate degree or two years of college, with four (4) years of healthcare experience; OR Bachelor's degree from a 4-year accredited college or university with two (2) years of healthcare experience
Preferred:
Master's degree a plus .
Experience:
Required:
Minimum two (2) years of experience working in a healthcare setting required. Experience with data management required.
Preferred:
Supervisory experience is strongly preferred.
Skills and Knowledge Required:
Ability to establish and maintain effective working relationships with co-workers, clients, and community partners. Ability to plan, organize, implement, evaluate, and modify program needs. Ability to independently solve problems and address issues as they arise. Ability to work with staff members across all levels of the agency. Computer and data-entry skills, including proficiency in Microsoft Suite.
Preferred:
Knowledgeable about Epic and its reporting features. Highly skilled at using Microsoft Excel. The Erie Advantage Pledge
