Job Description
Manager of Patient Programs
- Population Health Erie Humboldt Park Health and Dental Center

- Chicago, IL 60647 Apply Overview Salary Range $66,000.00
- $76,000.
- Population Health provides leadership, direction and strategic vision for quality initiatives across the organization to support UDS, HEDIS, agency and payor-related quality programs.
Retirement Programs:
401(k) program with Erie matching $0.50 for every $1.00 up to the first 5% of the employee's biweekly salary.Annual Paid Time Off:

Qualifications Education Required:
High school diploma or equivalent, with six (6) years of healthcare experience; OR Associate degree or two years of college, with four (4) years of healthcare experience; OR Bachelor's degree from a 4-year accredited college or university with two (2) years of healthcare experiencePreferred:
Master's degree a plus .Experience:
Required:
Minimum two (2) years of experience working in a healthcare setting required. Experience with data management required.Preferred:
Supervisory experience is strongly preferred.Skills and Knowledge Required:
Ability to establish and maintain effective working relationships with co-workers, clients, and community partners. Ability to plan, organize, implement, evaluate, and modify program needs. Ability to independently solve problems and address issues as they arise. Ability to work with staff members across all levels of the agency. Computer and data-entry skills, including proficiency in Microsoft Suite.Preferred:
