Healthcare Coordinator (Home Health)

Responsibilities Coordinate and schedule nursing and therapy tasks by collaborating with nurses, therapists, and other healthcare professionals to meet patient needs effectively. Serve as the main liaison between patients, families, and care teams to ensure clear communication and understanding of care plans. Maintain accurate and up-to-date documentation of patient information, visit details, and care activities within electronic health record systems. Assist in onboarding new patients by explaining services, answering questions, and ensuring all necessary paperwork is completed promptly. Monitor patient progress and follow up on any concerns or changes in condition to facilitate timely interventions. Support compliance with healthcare regulations and organizational policies through diligent record-keeping and reporting. Promote a positive patient experience by demonstrating empathy, professionalism, and proactive problem-solving at every interaction. Qualifications Proven experience in healthcare coordination, case management, or related roles within home health or outpatient services. Strong organizational skills with the ability to multitask efficiently in a fast-paced environment. Excellent communication skills—both verbal and written—to effectively liaise with diverse stakeholders. Knowledge of healthcare regulations, privacy laws (such as HIPAA), and electronic health record systems. Ability to work independently while maintaining a collaborative team approach.
