Job Description

Header Job Summary We are seeking a dynamic and dedicated Healthcare Coordinator to join our healthcare team. In this vital role, you will serve as a central point of contact for patient care coordination, ensuring seamless communication between medical providers, patients, and support services. Your expertise will help streamline discharge planning, manage patient intake processes, and facilitate comprehensive case management across diverse clinical settings. This position offers an exciting opportunity to make a meaningful impact on patient outcomes by coordinating care efficiently and compassionately. The ideal candidate is energetic, detail-oriented, and possesses a strong foundation in hospital and acute care environments. Responsibilities Coordinate patient treatment plans and walk them through scheduling. Manage patient intake processes, including verifying insurance coverage and collecting necessary documentation Facilitate utilization review and utilization management activities to optimize resource use and ensure compliance with healthcare standards Support case management efforts by developing individualized care plans for patients with complex needs. Maintain accurate documentation within electronic health record (EHR) systems while adhering to HIPAA regulations for patient privacy and data security Assist in coordinating services across various departments such as General Dentistry, Oral Surgery and Perio. Collaborate with multidisciplinary teams including doctor, dental assistant and labs to ensure comprehensive patient service delivery. Skills Critical care experience with a strong understanding of acute care protocols and dental operations Knowledge of EMR (Electronic Medical Records) systems and EHR (Electronic Health Records) platforms for documentation and communication Experience working within customer service/patient relations Strong understanding of HIPAA regulations to safeguard patient information at all times Excellent communication skills for effective collaboration with healthcare professionals and patients' Ability to navigate complex healthcare environments Great at multitasking in a fast paced environment Join us in delivering exceptional healthcare coordination that enhances patient experiences while supporting our dedicated clinical teams!
Pay:
$17.00 - $24.00 per hour
