Job Details:
The Outreach Coordinator PFK has the following essential job functions: Screens for healthcare needs and connects to related programs and resources. Interacts daily primarily via telephone and electronic communication. Initiates telephone contact and completes a health risk assessment. Escalates complex or urgent needs to a supervisor. Connects patients to the right program once needs are identified. Develops individualized self-management skills to patients and families through education and resource provision. Provides organization of electronic communication related to survey results of the health risk assessment. Documents within the electronic medical record. Participates in orientation and continuing education of staff and students as appropriate. Education High school diploma required Experience Background in health care, public health, or related clinical field preferred Skills/Competencies Excellent customer service skills with the ability to communicate effectively through multiple mechanisms including phone and writing required
