The CFSS Program Manager leads the day-to-day operations of the program, ensuring services run smoothly, efficiently, and in alignment with Hmong Home Health Care and regulatory expectations. This position manages the CFSS team members and provides leadership, guidance, and oversight to support high-quality client care. This role acts as a liaison across the organization and works closely with other programs and departments to ensure effective coordination and service delivery. THE ROLE
As the CFFS Program Manager, you will have the following responsibilities: Develop and update program policies and procedures Lead project management efforts, including development and implementation of new strategies Coordinate onboarding of new hires, including shadowing, system training, and delivery of program specific training Develop and update training materials Supervise and manage assigned staff and comfort coaching staff on complex situations Facilitate bi-monthly staff meetings and additional program meetings as needed Promote person-centered service delivery and HHHC values Build and maintain key internal and external relationships with partners such as case management, lead agencies, and community stakeholders Identify and leverage team strengths to improve performance Maintain professional and technical knowledge Represent the organization at industry events as needed Participate in internal and external committees and task forces Screen and assess candidates for Supervising Professional positions in partnership with leadership, as directed Provide operational coverage as needed Ensure compliance with DHS regulations and organizational standards Model leadership behaviors aligned with organizational values QUALIFICATIONS
Four-year degree and a minimum of 5 years of experience in a human service or health care related field Must meet DHS requirements to be a Supervising Professional (RN, Licensed Social Worker, or qualifying degree/experience combinations) Minimum of 2 years of management experience in a human services or health care field Demonstrated knowledge of services for people with disabilities and older adults, including agency model Community First Services and Supports (CFSS), person-centered service delivery, and applicable Minnesota DHS regulations Familiarity with DHS audits, documentation standards, and compliance risk for CFSS Strong critical thinking, creativity, and problem-solving skills Excellent organizational, communication, and interpersonal skills Knowledge of federal and state statutes related to services for people with disabilities and older adults in Minnesota Demonstrated ability to collaborate across teams as both a leader and contributor Strong verbal, written, and electronic communication skills Ability to adapt to rapid change Must pass a Minnesota Department of Human Services background study Valid driver's license, clean driving record, and reliable transportation Professional level proficiency in Microsoft Office tools Highly Preferred Qualifications Master's Degree in a human services-related field or Registered Nurse (State of MN) preferred Bilingual or multilingual abilities preferred (e.g., Spanish, Hmong, Laotian, Somali, or other languages reflective of the communities served) ADDITIONAL INFORMATION

This is an exempt position with a hybrid work model. Business hours are 9am - 5pm; Monday-Friday. Hmong Home Health Care is a highly collaborative and customer-focused work environment.
