Job Description
Company Summary CRISP Shared Services (CSS) serves as the technology partner for health information exchange (HIE) partners in Maryland, DC, West Virginia, Connecticut, Alaska, Virginia, Kentucky, Florida, and other jurisdictions. CSS facilitates the electronic transfer of clinical information between disparate information systems in order to positively impact the healthcare system through improved patient care and outcomes, reduced costs, and data-driven understanding of regional public health concerns. Job Summary The Health Related Social Needs Lead (HRSN) Lead / Project Manager is responsible for the strategy and implementation of health-related social needs initiatives in Maryland. This position works closely with project and policy leads at a fast-paced health data utility supporting HRSN policies and procedures for Maryland. They collaborate with the HRSN Outreach Liaison and Clinical Applications Trainer for stakeholder initiatives. They play a key role in the day-to-day management and operations of Public Health and HRSN work involving community-based organizations, hospitals, state and local health departments and more. Essential Duties and Responsibilities Include the following. Other duties may be assigned. Align Maryland Health Data Utility initiatives with HRSN priorities and policies Lead the implementation and operations of HRSN projects Serve as the lead Project Manager on all HRSN and related projects and tools Offer support to HRSN and public health providers across internal and external stakeholders to mitigate challenges Improve utilization of HRSN and public health tools in Maryland, including the CRISP referral tool, integrations, HRSN screeners, and reports Develop internal and external communication materials, and present HRSN use cases to relevant stakeholders Train external users on HRSN tools Participate in national groups, consult with medical professionals and policymakers to identify opportunities for the Health Data Utility to support HRSN interventions and programs Serves as subject matter expert on HRSN for internal staff including senior leadership Qualifications To perform this job successfully, the incumbent must be able to perform each essential duty satisfactorily. Demonstrated experience working with HRSN policy and programs across regions Familiarity with HRSN data policy, HRSN landscape, and closed loop referral tools Outstanding communication, organizational and interpersonal skills with both a technical and non-technical audience Strong project management, written and verbal communication skills Strategic thinker with ability to evaluate and solve complex problems and issues An understanding of how CBOs use electronic health systems Competence in navigating policy and data governance considerations and understanding their implications in HRSN initiatives Commitment to principles of health equity Strong attention to detail Experience and/or Education Requirements Bachelor's degree required; Master's degree preferred 5+ years of project management experience in social determinants of health, public health, or related field required Familiarity with health care policy and community-based organizations required Experience with agile process and JIRA preferred Compensations and Benefits Package Targeted base salary: $105,000 - $125,000 per year, based on experience and qualifications, plus benefits and bonuses. Benefits include the following but are not limited to health, dental, vision, life and disability insurance. CSS also has a 403(b) plan with discretionary employer match and contributions. You will be eligible for 20 days of Paid Time Off per year (accrued in accordance with CSS's policies). PTO is in addition to 8 company designated holidays. If you are not PTO eligible, you may be eligible for applicable state and/or local sick and safe leave in accordance with the provisions of any applicable state and/or local sick and safe leave laws.
