? 50-75% Local Travel Required Adecco Healthcare & Life Sciences is seeking a Fluent Bilingual Spanish/English Case Management Coordinator to support Medicaid Long-Term Care and Comprehensive Program members throughout the Mid Miami area. This is an excellent opportunity for a compassionate, organized professional who is passionate about improving healthcare outcomes and helping members navigate complex healthcare needs. This role offers the flexibility of working remotely while conducting face-to-face visits with members in the community. Candidates should be comfortable traveling approximately 50-75% of the time throughout the Mid Miami area. What You'll Do ? Coordinate case management activities for Medicaid Long-Term Care and Comprehensive Program members ? Conduct comprehensive member assessments using care management tools and clinical data ? Develop, implement, and monitor individualized care plans ? Coordinate care and services with healthcare providers, caregivers, and community resources ? Conduct multidisciplinary reviews to promote positive member outcomes ? Identify and escalate quality of care concerns through established processes ? Assist members in accessing benefits, services, and healthcare resources ? Utilize motivational interviewing and engagement techniques to encourage healthy lifestyle choices ? Provide education, coaching, and support to help members make informed healthcare decisions ? Advocate for members and assist them in navigating the healthcare system ? Maintain accurate documentation and ensure compliance with regulatory and accreditation requirements
