As a Member Navigator , you are at the heart of the Avenues experience, serving as a trusted advisor, advocate, and lifelong partner to our members. • $70,000 / year Reporting To Paige Hendrickson Key Responsibilities Member Support Provide personalized, one-on-one guidance to members, assisting them in understanding and accessing available healthcare and wellness services, benefits, and resources.

Build strong rapport and therapeutic relationships with members, their families, and other healthcare team members. Clearly communicate members' rights, options, and responsibilities regarding healthcare and wellness. Enhance members' and families' health literacy by explaining medications, medical terminology, insurance. Conduct comprehensive assessments to identify members' short

• and long-term needs, developing actionable strategies and goals to address those needs.

Oversee and coordinate caregivers and support staff in delivering high-quality care. Regularly monitor members' progress, address concerns, and adjust care plans as necessary. Other Responsibilities Accurately document member interactions and changing needs in the Electronic Medical Record (EMR) system to inform and coordinate with the care team. Maintain financial accountability in line with organizational standards. Participate in after-hours phone calls and, when needed, conduct after-hours home visits. Skills, Knowledge & Expertise Experience in healthcare navigation, case management, or as a medical/health professional is strongly preferred. Experience working with the senior population, required. Proficiency in medical terminology, required. A valid driver's license, reliable transportation, and proof of auto insurance are required. Bachelor's degree preferred, ideally in a related field. Strong interpersonal and communication skills, including active listening and the ability to show empathy.
