About the Role We are seeking a collaborative, highly organized, and mission-driven Program Manager to support Community Care Hub initiatives and Medicaid waiver-related programs focused on Health-Related Social Needs (HRSN). Reporting to the Senior Director of Programs, this role serves as a key liaison between Thriving Together NCW and community partners, helping coordinate implementation efforts, monitor contracts and deliverables, support statewide initiatives, and strengthen regional systems of care. Key Responsibilities Manage relationships with contracted partners and subcontractors Monitor contract deliverables, reporting requirements, and implementation progress Review invoices, budgets, reports, and supporting documentation for accuracy and compliance Coordinate partner meetings, communications, and problem-solving efforts Support Community Care Hub operations and Medicaid waiver initiatives Participate in regional and statewide collaborative efforts related to care coordination and Health-Related Social Needs Track project timelines, deliverables, and performance metrics Develop reports, presentations, and materials for leadership, partners, and funders Support cross-sector collaboration among healthcare, community, and public-sector stakeholders Qualifications Required Bachelor's degree in public health, public administration, healthcare administration, nonprofit management, social work, or related field (or equivalent experience) 3-5+ years of experience in program management, healthcare administration, nonprofit operations, project management, community-based services, or a related field Strong project management, organizational, and communication skills Ability to manage multiple priorities and work independently Experience building relationships with diverse stakeholders Proficiency with Microsoft Office applications Preferred Experience with Medicaid programs, healthcare transformation initiatives, or Health-Related Social Needs programs Experience reviewing contracts, grants, budgets, invoices, or compliance documentation Familiarity with Community Care Hubs, Community Information Exchange (CIE), or care coordination initiatives Why Join Us?
