Job Summary We are looking for a motivated Rural Health Transformation Project Manager ready to take us to the next level! Job Description We are looking for a Rural Health Transformation Project Manager to support Indiana's Rural Health Transformation Program (RHTP) Initiative 6: Growing Access to Hospital Post Discharge Medications. This position will be Full-Time and Hybrid, working onsite three days per week at the Indiana Department of Health (IDOH) office in downtown Indianapolis, with the option to work remotely two days per week. Only candidates without a need for sponsorship now or in the future will be considered. Candidates must currently be a commutable distance to downtown Indianapolis. What You'll Do Coordinate and facilitate Meds to Beds programs with rural hospitals throughout Indiana and other key stakeholders Foster partnerships with rural hospital administrators to generate hospital buy-in and overcome program implementation barriers Ensure hospitals receiving grant funds to develop Meds to Beds programs meet deliverables outlined in their work plans and adhere to contract requirements Develop relationships with vendors such as medical record systems, couriers, pharmacies, Pharmacy Benefits Managers (PBMs), and Pharmacy Services Administration Organizations (PSAOs) Provide onsite technical assistance and serve as a liaison between participating hospitals, technical support resources, and IDOH leadership Work with hospital CEOs, pharmacy leadership, and external partners such as the Indiana Pharmacy Association (IPA) Share best practices from successful Meds to Beds programs with other hospitals and health systems Develop reporting guidelines and systems to assess program outcomes Provide technical support to hospital staff responsible for tracking project-related data and outcomes Coordinate measurement, analysis, and reporting of progress on defined Key Performance Outcomes (KPOs) with IDOH leadership and provide regular feedback Regular travel within the state (approximately 30-40%), with occasional out-of-state travel for conferences and meetings (1-2 times per year) Perform other duties as assigned
What You'll Need Required:
A medical or pharmacy-related degree, or a degree in public health, health science, health policy/administration, social science, or another relevant field Ability to plan, prioritize, and complete multiple projects simultaneously for optimal results Ability to synthesize knowledge gained from analytic functions into rational, objective, and cohesive policy recommendations Excellent written and oral communication skills, including the ability to present information to large and diverse audiences Ability to establish and maintain effective working relationships with agency staff, hospital administrators, and other key stakeholders Proficiency with Microsoft Office software (Word, Excel, PowerPoint, Publisher) Detail oriented and self-motivated
Preferred:
Minimum of two (2) years of experience in rural healthcare initiatives Clinical background with experience in pharmacy, hospital, primary care, ambulatory, or community healthcare settings Knowledge of federal and state pharmacy laws, policies, and procedures Education and experience in leadership, facilitation, adult education, and project management Familiarity with Pharmacy Benefits Management (PBM), Pharmacy Services Administration Organizations (PSAOs), and point-of-sale pharmacy systems Physical Demands Ability to safely and successfully perform the essential job functions consistent with ADA and other federal, state, and local standards Ability to conduct repetitive tasks on a computer, utilizing a mouse, keyboard, and monitor Must be able to communicate with customers/team members over the phone and in person Sedentary work that involves sitting or remaining stationary most of the time, with occasional need to move around the office to attend meetings #LI-JH Reasonable accommodation statement If you require a reasonable accommodation in completing this application, interviewing, completing any pre-employment testing, or otherwise participating in the employment selection process, please direct your inquiries to or (888) 824 - 8111. $58.00 $63.00 per hour The pay range for this position is listed above.
