Job Description

Lakewest Rehabilitation and Skilled Care is looking for an experienced MDS nurse to join its dynamic nursing team. The ideal candidate will be an LVN, possess strong organizational skills, be responsible for notifying and coordinating with the interdisciplinary teams for MDS assessment completion in accordance with State and Federal regulations. Candidates must also have at least 1 year of MDS and PASRR experience.
