Job Description

Job Summary Join our compassionate team as a Hospice Aide (PCA) providing in-home care to patients facing life-limiting illnesses. In this vital role, you will deliver personalized support that enhances comfort, dignity, and quality of life for patients in their own homes. Your dedication will help families navigate challenging times with empathy and professionalism, ensuring each patient's unique needs are met with kindness and expert care. This position offers a rewarding opportunity to make a meaningful difference in the lives of individuals and their loved ones. Duties Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary. Monitor and document patient condition through observation of vital signs, mood, and physical status, reporting changes promptly to the healthcare team. Provide medication reminders and assist with medication administration following medical orders while maintaining compliance with HIPAA regulations. Support patients with specialized care needs including Alzheimer's, dementia, memory care, and hospice-specific protocols. Perform light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe environment tailored to patient needs. Maintain accurate medical documentation within Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to ensure seamless communication among care providers. Engage in patient education on health maintenance, safety measures, and comfort strategies to promote well-being at home. Experience Prior experience in home health care or assisted living environments preferred; experience in nursing homes or long-term care settings is highly valued. Familiarity with medical terminology, medical records management, and documentation standards such as HIPAA compliance. Hands-on experience working with individuals with disabilities, developmental disabilities, or those requiring specialized memory care including Alzheimer's or dementia care. Demonstrated ability to work independently while collaborating effectively within a multidisciplinary healthcare team. Strong skills in patient observation, vital signs measurement, and use of assistive devices like Hoyer lifts or mobility aids.
