In this vital role, you will provide compassionate, personalized care to promote independence, safety, and well-being. Whether supporting residents in group homes, assisted living facilities, or home and community settings, your energetic and proactive approach will foster a nurturing environment where individuals thrive. Duties Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary. Monitor residents' health by taking vital signs, observing for changes in condition, and reporting findings accurately while maintaining HIPAA confidentiality. Prepare nutritious meals aligned with individual care plans and dietary restrictions; ensure cleanliness of the dining area and kitchen. Administer medications following prescribed protocols and document medication administration meticulously to ensure compliance with healthcare regulations. Support residents with memory care needs such as Alzheimer's or dementia by implementing specialized care strategies and engaging them in meaningful activities. Maintain a safe environment through regular cleaning, sanitation, and adherence to safety procedures; assist with light housekeeping tasks as needed. Collaborate with healthcare professionals, social workers, and family members to develop and update personalized care plans that meet each resident's unique needs. Requirements Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred. Knowledge of patient observation techniques, vital signs measurement, medication administration, and emergency response procedures such as CPR and first aid. Experience caring for individuals with developmental disabilities, autism spectrum disorder (ASD), Alzheimer's disease, dementia, or hospice needs is advantageous. Ability to perform heavy lifting safely and operate assistive devices like Hoyer lifts or resident lifts when necessary. Strong communication skills to effectively work with residents, families, healthcare teams, and community resources. Demonstrated understanding of HIPAA regulations to protect resident privacy and confidentiality. Flexibility to work various shifts including evenings, weekends or overnight hours as required for 24/7 care environments.
