Job Description

Rise Above Support Services is seeking qualified and dependable Caregivers/Direct Support Professionals (DSPs) to provide services to members enrolled in the Arizona Division of Developmental Disabilities (DDD). This position supports individuals with developmental disabilities in maintaining independence, health, and safety within their homes and communities in accordance with individualized service plans. This role requires a high level of professionalism, reliability, and adherence to state and agency guidelines. The ideal candidate is compassionate, attentive, and committed to delivering person-centered care. Key Responsibilities Provide direct care services in alignment with each member's Person Centered Plan (PCSP) Assist with activities of daily living (ADLs), including personal care, hygiene, and mobility Support members in community integration activities (appointments, outings, social engagement) Implement habilitation goals, Attendant Care tasks and Respite Services as outlined in service plans Assist with meal planning and preparation in accordance with dietary and health needs Perform light housekeeping to ensure a safe and sanitary environment Accurately document services provided and report any changes in condition or incidents per DDD requirements Maintain compliance with Electronic Visit Verification (EVV) requirements, including clocking in and out in real time via a mobile device in accordance with federal law Maintain professional boundaries while offering respectful companionship and support Follow all health, safety, and emergency protocols, including mandated reporting requirements Minimum Qualifications High school diploma or equivalent required Current Article 9 certification (Arizona required training on member rights and behavioral treatment) (or ability to obtain prior to hire) Experience working with individuals with developmental disabilities or in a caregiving role preferred Current CPR/First Aid certification (or ability to obtain prior to hire) Ability to pass all required background checks and obtain a valid Level 1 Fingerprint Clearance Card (Arizona requirement) Valid driver's license and reliable transportation (if required for the role) Access to a smartphone or mobile device capable of supporting EVV compliance Ability to complete all required DDD and agency training Knowledge, Skills, and Abilities Understanding of person-centered care and HCBS principles Strong communication and documentation skills Ability to follow care plans and behavioral support strategies Professionalism, reliability, and strong ethical standards Ability to manage time effectively and work independently Physical ability to assist with mobility and daily care tasks

$17.30 - $19.00 per hour
