Job Description

Job Summary At My Daughter's Home Health , we believe care should feel personal, calm, and respectful . We are a nurse-led home health agency looking for a compassionate and dependable Home Health Aide (HHA) , where you will play a vital role in providing compassionate, personalized care to individuals in their own homes. This role is about presence, trust, and consistency . You'll make a meaningful difference in the lives of patients by assisting with daily activities, ensuring safety, and promoting independence. As a Home Health Aide, you will work closely with clients, families, and healthcare professionals to deliver high-quality home and community care that enhances well-being and supports recovery or ongoing health management. This is a paid position with flexible scheduling and the opportunity to grow within a supportive, heart-centered team. This role is for someone who: Enjoys one-on-one patient care Understands that home health is about relationships , not rushing Is reliable, kind, and emotionally aware Feels comfortable working independently in patients' homes Takes pride in doing the small things well Responsibilities Provide assistance with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary Monitor patient health by observing vital signs and reporting changes to healthcare providers promptly Assist with medication administration following prescribed schedules and protocols while maintaining accurate medical documentation Support patients with memory care needs, including Alzheimer's and dementia care, ensuring a safe and comforting environment Utilize Electronic Health Record (EHR) systems to document patient care activities accurately and securely in compliance with HIPAA regulations Perform light housekeeping duties such as cleaning, laundry, and meal preparation to promote a clean and healthy living space Assist with medical procedures including catheter care, wound care, and hospice support as trained and required Requirements Prior experience in assisted living facilities, nursing homes, or home health environments is highly preferred Strong skills in patient observation, medical terminology, and caregiving techniques specific to senior care and individuals with disabilities or developmental disabilities Certification in CPR is required; additional certifications such as medication administration, hospice care, or dementia training are a plus Ability to operate EMR (Electronic Medical Records) or EHR systems efficiently for documentation purposes Knowledge of infection control practices including HIPAA compliance for patient privacy and confidentiality Physical ability to perform caregiving tasks including lifting using Hoyer lifts and assisting with mobility needs Excellent communication skills to collaborate effectively with healthcare teams and family members Why Work With Us Nurse-led leadership that supports and trusts HHAs Calm, respectful work culture Meaningful relationships with patients and families Flexible scheduling Opportunity to grow as the agency grows We believe: Care at home should feel safe and human Patients deserve time and kindness CNAs play a critical role in preventing complications and hospitalizations Doing healthcare differently starts with treating caregivers well Join us in delivering exceptional home health services that empower individuals to live comfortably and safely in their own homes.
