At MERCI, we believe in creating opportunities for our participants to lead meaningful lives while embracing their individuality and preferences. As a member of the MERCI team, you'll play a pivotal role in our Discovery Program, reaching out to individuals who, due to medical fragility, are unable to access our on-site campus regularly. In this role, you'll provide personalized 1:1 support, assisting with daily living activities such as feeding and personal care. You'll be integral in implementing tailored care plans, ensuring adherence to dietary and medical needs, and fostering a proactive, positive environment where participants thrive.What You'll Do:
Work closely with individuals in the Discovery Program, providing 1:1 support in their homes. Assist participants in integrating into the community, enabling them to engage in work, volunteer opportunities, and social activities. Respect individual preferences and rights, allowing participants to shape their own schedules and activities. Support participants in taking ownership of their services and support, empowering them to lead fulfilling lives. Foster successful community integration while recognizing when to step back appropriately. Maintain accurate records and reports. Attend team meetings, training sessions, and in-service programs.What You Need:
High school diploma or equivalent. Proficiency in English communication, both verbal and written. First Aid and CPR certification (training provided if needed). Ability to lift up to 50 pounds. Successful completion of health check, including TB test (costs covered). Clear background check, including fingerprinting. Valid California Driver's License and insurance. Reliable vehicle.
