Job Description

Description:
We are looking for a Medical Social Worker to provide care at our Patient's homes in primarily Solano County, and some Contra Costa Counties. As a Medical Social Worker, you will assist patients and families in coping with problems resulting from severe or long-term illness. You will assess, diagnose, and treat patient's mental and social conditions, counsel individuals and/ or families, and update case records.
Requirements:
Assesses psychosocial status of patient and social/ emotional factors related to the patient's illness, need for care, and response to treatment. Assisting patients and families with goal setting, counseling, and planning and providing interventions. Assisting patients and families with appropriate community recourses, their eligibility criteria, and regulations. Participates in the development and periodic renewal of the patient's plan of care. Observes, and reports to the physician the response to the patient's treatment. Maintains verbal and written communication with other personnel involved with the care of the patient. Maintains appropriate and timely patient care records in the EMR, including observation, treatments, patient response to treatment, and change in patient's condition Attends and participates in appropriate patients care conferences, staff meetings, and agency committee meetings.
