Full Job Description Job Description:
The Wellness Assistant plays a vital role in supporting individuals in an adult day program by assisting with integrating clients into the broader social environment. This includes facilitating activities that promote physical, emotional, and social wellness, while ensuring a safe and engaging atmosphere for participants. The role also involves transporting clients to and from various locations and assisting with wellness tasks, such as helping with activities and warming up meals. The specific job title and compensation will depend on the candidate's experience, background, and education.Key Responsibilities:
Transport Clients:
Safely drive clients to and from the adult day program, ensuring timely arrivals and departures, and assisting clients with entering and exiting the vehicle as needed. Be punctual and maintain high standards of professionalism.Facilitate Wellness Activities:
Support clients in participating in physical, cognitive, and recreational activities, including exercise, games, and social events that encourage socialization and engagement.Social Integration Support:
Help clients feel comfortable and included in group settings, fostering positive interactions and relationships among participants.Direct Care and Supervision:
Provide direct care and supervision of clients throughout the day, ensuring their safety and well-being. Actively encourage clients to be as independent as possible while assisting them with tasks when necessary.Monitor Client Well-Being:
Observe clients for any functional changes or changes in health status, including physical, emotional, or behavioral shifts. Report these changes to the team lead promptly.Meal Assistance:
Assist clients by warming up meals, ensuring they are served in a comfortable and timely manner. Provide support during mealtime as needed, ensuring clients' safety and comfort.Hygeine Care :
Help with bathroom use, hand washing, changing diaper, accidental soiling/spillsHousekeeping:
Cleaning bathrooms, vacuuming, cleaning tables/chairs, maintaining activity room.Encourage Participation:
Actively engage clients in discussions, games, and interactive activities, promoting a sense of community and helping clients feel valued and supported. Maintain aSafe Environment:
Ensure that all areas are safe, clean, sanitary, and free of hazards, and follow all safety protocols to maintain a safe setting for both clients and staff. Ensure Compliance withISPs:
Be familiar with and ensure that clients' Individual Service Plans (ISPs) are followed. Support the implementation of care plans and activities that align with the specific needs and goals of each participant.Documentation and Reporting:
Accurately document client participation, activities, and any changes in their physical or emotional condition. Provide detailed reports to the team lead regarding observed functional changes, and ensure all records are up-to-date and compliant with company policies.Team Collaboration:
Work closely with other program staff, including healthcare professionals, to ensure clients' needs are met and care plans are followed.Manage Escalations:
Respond appropriately and safely to behavioral escalations from clients with developmental disabilities, including situations that may involve verbal outbursts or physical behavior. Utilize approved de-escalation techniques and follow protocols to ensure the safety and dignity of all involved.
