We are currently seeking a compassionate, organized, and team-oriented individual to join our Home Health team as a Part Time (Per Diem) Home Health Aide (HHA) in Brentwood, Antioch, Oakley, Discovery Bay. Position Overview:

We are seeking an experienced, self-driven Part Time (Per Diem) Home Health Aide (HHA) to join our dedicated Home Health team and provide exceptional care throughout the region. As an Home Health Aide (HHA), you will evaluate patients' health, create and implement care plans, and track their progress to maintain the highest standards of care. You'll be working closely with our Clinical Director and Clinical Staff, providing support both remotely and independently in the field. Requirements:

Home Health Aide (HHA) License to practice in California

• Required Experience in Home Health or Hospice care
• 1 Year Required Current CA Driver's License. Able to travel within a 15
• 25 mile radius on a regular basis.
