Full Job Description Job Overview:

Shift:

Monday to Friday 6 hours per day 9am to 3pm. We are a community-based day program. We are seeking dedicated individuals to join our team as Direct Support Professionals/Coaches. As a coach, you will provide essential support to individuals with developmental disabilities while out in the community.

Duties:

• Assist individuals through guidance, support and coaching
• Ensure the safety and well-being of individuals under your care
• Implement ISP goals
• Maintain accurate documentation
• Advocate for individual's rights and promote independence
• Provide direct coaching services as outlined in the Individual Placement Plan (IPP) including one-on-one training of specific skills
• Work closely with Program Supervisor to monitor Individual Services Plans.
• Work on personalized goals as per IPP.
• Work with the Program Supervisor to coordinate assessments, arranging for the development of individualized behavior support plans as needed.

Requirements:

• High School Diploma/GED
• Being able to work out in the community
• Must have a valid Drivers license and a good driving record
• Must pass DOJ/FBI background check
