This position offers an exciting opportunity to make a meaningful difference in patients' lives by assisting with daily activities, managing medical needs, and supporting overall well-being. As a CHHA, you will work closely with clients, families, and healthcare professionals to deliver high-quality care that promotes independence and comfort. Responsibilities Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts. Monitor and document patient health status by taking vital signs and recording medical information accurately in Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems. Administer medications and manage treatments following physician instructions while ensuring compliance with HIPAA regulations to protect patient privacy. Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory impairments, including memory care techniques and behavioral management. Support patients with complex medical needs such as catheter care, hospice care, and working with individuals with developmental disabilities or disabilities. Maintain a clean and safe environment by assisting with light housekeeping tasks like laundry, cleaning, meal preparation, and ensuring proper sanitation practices. Communicate effectively with healthcare teams regarding patient progress, changes in condition, or concerns related to patient care. Skills Proven experience in home health or long-term care environments, including assisted living or nursing home settings. Strong knowledge of medical terminology and clinical procedures such as vital signs monitoring and medication administration. Skilled in patient observation to detect changes in condition or behavior promptly. Proficiency with EMR/EHR systems for accurate medical documentation and record keeping. Ability to work independently while following detailed care plans and adhering to safety protocols like CPR certification. Experience working with individuals with disabilities or special needs, including dementia or Alzheimer's care. Excellent caregiving skills combined with compassion, patience, and strong communication abilities to foster trust and rapport with clients.
