Description:
Home Health / Hospice Aide Join Saint Agnes Home Health and Hospice , part of Trinity Health At Home, and make a meaningful impact by providing compassionate care to clients in their homes.
Responsibilities:
Assist with personal care (bathing, grooming, dressing, mobility) Light housekeeping, meal prep, and laundry Monitor vital signs and report changes Support medication self-administration and equipment safety Maintain accurate documentation and communicate with RN supervisor
Qualifications:
High school diploma/GED Must be a Certified Home Health Aide (HHA) in the state of California Federally compliant training and competency evaluation 1+ year experience in home care, hospice, or institutional setting Driver's license and reliable transportation Ability to lift up to 35 lbs
