Job Description

The Direct Support Personnel II assists individuals with intellectual disabilities or mental health challenges with growth and development of interpersonal skills and activities of daily living in the home, community or residential setting. The Direct Support Personnel II works directly with the resident and provides supports for activities of daily living and adaptive skills. The Direct Support Personnel II should follow the guidance of the resident/ patients Individual Service Plan.
  • Minimum Requirements:
  • + High School diploma or its equivalent + Valid driver's license + Valid and current auto insurance + Complies with all relevant professional standards of practice + Current CPR if applicable + TB questionnaire, PPD or chest x-ray if applicable + Current Health Certificate (per contract or state regulation) + Must meet all federal, state and local requirements + Must be at least 18 years of age + Demonstrates ability to prioritize tasks •Position Details•

Medications:
1. Administering medications 2. Shadowing and coaching staff on how to administer medications 3. Assisting with Cycle Fill
  • Monthly 4. Monitoring med cart
  • ensuring that MARS and meds and doctors orders are accurate
Client Care:
  • Client assessment/intervention/develop care plan and medical protocols as necessary 1.
Provide staff education/coaching/training to ensure correct understanding and proper execution of medical intervention, protocols 2. Client assessment and determination as to whether or not to transport to urgent care/ER 3. Carry out orders; Staff teaching/training on new ordered treatments as necessary; Relaying information to treatment team and family member as appropriate 4. The nurse is part of the staff ratios and are required to assist with toileting, feeding,
ADL Documentation:
1. Recording medical health information in Profiler
  • electronic medical records system 2. Completing consult sheets 3. Filing documentation in charts 4. Completing incident reports
  • SIRs 5. Completing RADAR
  • Devereux incident report electronic system 6.
Communication with EOD and Program teams via e-mail 7. Medication destruction 8. Charting in medical binder
Phones and Communication:
1. Answering landline
  • picking up voicemails 2. Communication with EOD and program team members 3. Communication with family members
  • Relaying information to treatment team and family member as appropriate 4. Communications with medical providers physicians, pharmacy, treating specialists
  • Ensuring that accurate information is given to avoid confusion, duplication of services and serious medical errors and appropriate treatment is provided; carry out orders; Staff teaching/training/coaching on new ordered treatments as necessary 5.
Coordination/communication of treatment/medical needs with outside nursing agencies 6. Design and implementation of protocols for individual medical need beyond first aide as approved by treating physician 7. Training of staff in protocols for individual medical need beyond first aide as approved by treating physician 8. Monitoring and updating of care instructions for individual needs beyond first aide.
