In this role, you will provide compassionate, personalized care to residents in group homes, assisted living facilities, or home and community settings. Your energetic and proactive approach will help create a safe, supportive environment where each individual can thrive. This paid position offers an opportunity to make a meaningful difference every day while developing valuable skills in patient care and support. Responsibilities Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts or resident's Monitor and document residents' vital signs, health status, and behavioral changes to ensure safety and well-being Support medication administration following HIPAA guidelines and care plans developed by healthcare professionals Prepare nutritious meals and assist with meal planning, cooking, and cleaning to promote healthy living Conduct patient observation to identify needs or concerns promptly and communicate effectively with healthcare teams Engage residents in social activities, recreational programs, or community outings to foster social interaction and independence Maintain a clean, safe environment by performing light housekeeping duties aligned with health standards for home & community care settings Experience Prior experience working in group homes, assisted living, nursing homes, or home health environments preferred Hands-on experience with patient monitoring, dementia care, Alzheimer's care, or memory care is highly valued Knowledge of working with individuals with developmental disabilities or autism enhances your ability to provide tailored support Certification in CPR and First Aid is required; additional certifications such as HIPAA training are a plus Experience with medication administration, care plans development, and patient observation is essential for success in this role Demonstrated ability to lift heavy objects safely using equipment like Hoyer lifts and perform physical tasks required for resident assistance Background in social work or nursing enhances understanding of holistic patient care approaches Join us to make a positive impact on the lives of those we serve!
