In this vital role, you will provide personalized care, support daily activities, and foster a safe, respectful environment. Duties Assist individuals with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary. Monitor residents' health by taking vital signs, observing patient conditions, and reporting changes promptly to healthcare professionals. Prepare nutritious meals tailored to dietary needs and assist with meal planning and cooking to promote healthy living. Support medication management Maintain a clean, safe environment by performing regular cleaning duties within residents' living spaces and common areas. Engage clients in social activities, community outings, and recreational programs to enhance their quality of life. Document care activities accurately in accordance with facility policies, including care plans, incident reports, and patient progress notes. Follow all safety protocols including CPR and first aid procedures; respond effectively during emergencies or incidents. Support residents with specialized needs such as dementia or Alzheimer's care, memory care, autism spectrum disorders, or hospice care as required. Use patient observation skills to monitor behavioral changes or health concerns and communicate effectively with team members. Experience Prior experience working in group homes, assisted living facilities, nursing homes, or home & community care environments is highly desirable. Demonstrated knowledge of patient care techniques including assisting with ADLs, medication administration, and vital signs monitoring. Experience with specialized populations such as individuals with developmental disabilities, dementia or Alzheimer's disease, autism spectrum disorder, or those requiring hospice or memory care is preferred. Familiarity with safety procedures including CPR and first aid certification is essential; additional certifications in HIPAA compliance or nursing assistance are a plus. Ability to lift heavy objects safely and perform physical tasks required for resident support and mobility assistance. Strong communication skills for effective collaboration with team members, clients, families, and healthcare professionals.
