In this vital role, you will foster independence, promote well-being, and ensure safety through personalized assistance in residential, community, or home care settings. Duties Assist individuals with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary. Monitor patients' vital signs and observe their condition closely to identify any changes or concerns promptly. Administer medications following care plans and HIPAA regulations to ensure safety and compliance. Prepare nutritious meals tailored to dietary needs and assist with cooking and meal planning. Maintain a clean, safe environment by performing regular cleaning tasks in accordance with health standards. Support residents' participation in community activities, outings, and social events to promote engagement and independence. Document care activities accurately in compliance with HIPAA guidelines and update individual care plans regularly. Respond quickly and effectively during emergencies by utilizing CPR and First Aid skills when needed. Collaborate with team members, families, social workers, nurses, and other professionals to deliver holistic care tailored to each individual's needs. Skills Proven experience working in group homes, assisted living facilities, nursing homes, or home & community care environments. Strong knowledge of patient observation techniques and vital signs monitoring. Skilled in assisting with ADLs, medication administration, and implementing care plans for individuals with disabilities or dementia-related conditions. Familiarity with HIPAA regulations to ensure confidentiality and privacy of patient information. Ability to perform heavy lifting safely using equipment like Hoyer lifts when required. Excellent communication skills for working effectively with individuals with developmental disabilities or autism spectrum disorder. Experience in providing Alzheimer's care or memory support is highly valued. Capable of maintaining a clean environment through regular cleaning tasks while adhering to health standards. Knowledge of emergency procedures including CPR and First Aid certification is preferred.
