Job Description

Overview Join our compassionate team as a Caregiver Home Health Aide and make a meaningful difference in the lives of individuals requiring personalized care and support. In this vital role, you will provide attentive, respectful assistance to clients in their homes or care facilities, promoting independence and enhancing quality of life. Your dedication will help clients maintain comfort, safety, and dignity while receiving the highest standard of care. This position offers an opportunity to work closely with diverse populations, including seniors, individuals with disabilities, and those receiving hospice or memory care services. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using equipment such as Hoyer lifts. Monitor patient health by observing vital signs, recognizing changes in condition, and reporting findings accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems. Support medication administration following proper medical terminology and HIPAA privacy regulations to ensure safety and compliance. Provide specialized care for clients with dementia or Alzheimer's disease, implementing memory care strategies to promote comfort and safety. Assist with medical procedures such as catheter care, wound management, and hospice support as trained. Prepare nutritious meals and assist with feeding while adhering to dietary restrictions and preferences. Maintain a clean and safe environment by performing light housekeeping tasks like laundry, cleaning, and organizing to ensure a hygienic living space. Document all patient interactions accurately in medical records or documentation systems to ensure continuity of care. Use EMR/EHR systems efficiently for scheduling, documentation, and communication with healthcare teams. Support clients' emotional well-being through compassionate communication and engagement tailored to individual needs. Skills Strong patient observation skills to detect subtle changes in health status promptly. Experience working in assisted living facilities, nursing homes, or home & community care environments. Knowledge of medical terminology and procedures relevant to home health care settings. Ability to operate medical equipment such as Hoyer lifts, blood pressure cuffs, thermometers, and other vital sign monitors. Familiarity with caring for individuals with developmental disabilities or dementia-related conditions like Alzheimer's disease. CPR certification and training in emergency response protocols to ensure client safety at all times. Experience with medication administration, including understanding prescriptions and proper handling procedures. Proficiency in working with electronic documentation systems like EMR/EHR platforms while maintaining HIPAA compliance. Compassionate caregiving skills combined with excellent communication abilities for working effectively with clients and their families. Ability to perform light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe environment for clients requiring long-term or live-in care.
