Full-Time Home Health Marketer / Community Liaison & Administrative Support Precise Med Home Health - Roseville / Greater Sacramento Area Precise Med Home Health is actively expanding and seeking a driven, professional, and relationship-oriented Full-Time Home Health Marketer / Community Liaison & Administrative Support team member to support the continued growth of our organization throughout the Greater Sacramento region. This position is ideal for someone who enjoys relationship-building, community outreach, and healthcare networking while also being comfortable assisting with light office and administrative support during slower outreach periods. As a representative of Precise Med Home Health, you will help strengthen referral relationships, increase community presence, support patient growth, and assist with daily office operations. Primary Focus AreasPhysician Office Outreach Build and maintain relationships with primary care physicians, specialists, and clinic staff Conduct routine in-person visits to medical offices Present Precise Med Home Health services professionally and effectively Maintain consistent communication and follow-up with referral sources Increase agency visibility within the medical community Skilled Nursing Facility & Assisted Living Outreach Network with Administrators, Directors of Nursing (DONs), Social Services Directors, Case Managers, and discharge planners Support discharge planning and transitional care efforts Establish strong facility partnerships through regular visits and relationship management Deliver marketing materials and maintain ongoing presence within facilities Community Engagement & Business Development Represent Precise Med Home Health at community events, healthcare gatherings, and networking opportunities Build relationships with senior care professionals, care managers, and healthcare partners Identify opportunities to expand community presence and referral partnerships Referral Growth & Relationship Management Develop and maintain new referral relationships Strengthen and support existing referral partnerships Communicate service availability, coverage areas, and operational updates Track outreach efforts and provide activity reporting to leadership Administrative Office Support Provide light administrative office support during slower outreach periods and afternoons Assist with organizing paperwork, answering calls, follow-up communication, and coordination with the intake team Help support daily office operations while maintaining flexibility between field marketing and office responsibilities General Responsibilities Conduct scheduled field visits throughout the week Perform warm follow-ups with referral partners and healthcare contacts Maintain organized outreach documentation and activity logs Communicate professionally in both in-person and digital interactions Coordinate closely with intake and clinical teams to support smooth patient onboarding Maintain a polished, professional, and dependable presence while representing the company Qualifications Strong communication and interpersonal skills Professional, confident, and relationship-driven presence Organized, dependable, and self-motivated Healthcare marketing, sales, community liaison, or administrative experience preferred, but not required Ability to work independently and professionally in both field and office settings Reliable transportation required Compensation & Benefits $24-$25 per hour

Performance Bonus:

$50-$75 per admission Full-time position (40 hours per week) Flexible scheduling Medical, Dental, and Vision Insurance available after 60 days of employment Hourly rate listed in compliance with Indeed posting requirements.
